Forms
Course Reserve Form – If you wish to place material on reserve for your class, please complete this form, and submit it to the Circulation Desk along with the material that is to be placed on reserve. The form is necessary so that library staff can determine the correct course, instructor, and loan period.
Rush Request Form – Occasionally, you will encounter a message on OSCAR, which states that the material you need is "being processed for Heafey." If this message appears on an OSCAR record, it means that library staff have obtained the item for the library's collection, but the item is still being processed by our technical services staff, and it is not on the shelf yet. Patrons can request rush processing of "in process" items by filling out this form, attaching a copy of the item's OSCAR record, and submitting it to the Circulation Desk or a reference librarian. It usually takes five business days, excluding weekends and holidays, to process rush requests. A member of the library staff will contact you when the item is available. If an OSCAR record states that an item is "on order," you should request the item through LINK+ or Interlibrary Loan because the library staff cannot guarantee when the ordered item will arrive.
Law Research Assistant Faculty Check-Out Form – Law Student Research Assistants who wish to borrow material for a faculty member or transfer items previously checked out to their student account to a faculty account must fill out and submit a faculty signed form to the Circulation Desk. Law Student Research Assistants are responsible for any overdue fines or charges on items that are not transferred to a faculty account.
Library Release Forms -- These forms are used for students completing the writing requirement for a certificate. By filling out this form, students enable the library to add their written work into the library's collection. There is a separate form for the High Tech Law Certificate, Public Interest and Social Justice Certificate, and the International Law Certificate.
